How we collect your personal information
We collect most of your personal information from you.
This is collected at predominantly four locations:
- when you visit an OPSM for an eye test.
- when you visit an OPSM to purchase eyewear and associated products.
- when you visit our website and subscribe to join our OPSM mailing list for news and offers.
- when you order contact lenses from OPSM.
At each of these collection points you will be required to provide your name, address, and contact details. Your OPSM optometrist may also ask questions on lifestyle, occupation and hobbies to assist in choosing the best eye care and eyewear options available.
There may also be occasions when we need to obtain personal information from a third party. For example, we may collect personal information from your health fund or from a government agency such as the Department of Veterans Affairs. These third parties will advise to us of your eligibility to rebates on eye care services and eyewear products.
How we use your personal information
In providing you with our services we need to collect personal information from you, which is necessary for us to perform a thorough service and provide you with the most suitable eye care. Your OPSM optometrist has access to your records for professional purposes only.
We collect your personal information to provide you with the most suitable eye care services and eyewear product.
We may also use your contact details to:
- advise you on your eye care needs including when you need another eye test.
- advise you if we relocate.
- seek your input on your service from OPSM.
- keep you updated with new eyewear products and services and special promotions and offers.
Our optometrists, with your consent, may refer you to an eye doctor and forward relevant information to that medical practitioner about your particular condition.
We keep your personal information up-to-date
We require accurate personal information about you to ensure you receive the best possible service from OPSM.
We take reasonable steps to ensure that your personal information is up-to-date whenever we collect it or use it.
We will correct personal information
If you find that current personal information we hold about you is inaccurate or out-of-date, please contact us immediately and we will correct it.
You can access your information
OPSM will on request provide you with information about you, which OPSM holds.
This information can be accessed by calling the OPSM Help Desk on 1800 626 300 during business hours. Please ensure you are able to advise which retail outlet you last visited for eye care services and eyewear product.
Your request for information will be dealt with within a reasonable timeframe. A small charge may be applicable to cover the administration costs involved in providing this information to you.
We store your personal information securely
We protect any personal information that we hold from misuse and loss. We also protect if from unauthorised access, modification and disclosure.
Only authorised users can access your personal information, and access is only for approved purposes.
Your personal information may be stored in hard copy documents or electronically.
We maintain physical security over our paper and electronic data storage. We also maintain computer and network security for example, we use firewalls (security measures for the Internet) and other systems such as user identifiers and passwords to control access to computer systems.
All our employees are bound by the highest principles of confidentiality, severe breach of which is penalised by dismissal.
