How we collect your personal information
We collect most of your personal information from you.
This is collected at predominantly three locations:
- when you visit an OPSM associated Optometrist for an eye examination
- when you visit an OPSM retail outlet to purchase eyewear and associated products
- when you order contact lenses from OPSM Direct via the telephone, mail, email, facsimile or through our website
At each of these collection points you will be required to provide your name, address, and contact details. Our Optometrist or Optical Dispenser may also ask questions on lifestyle, occupation and hobbies to assist in choosing the best eyecare and eyewear options available.
There may also be occasions when we need to obtain personal information from a third party. For example, we may collect personal information from your health fund or from a government agency such as the Department of Veterans Affairs. These third parties will advise to us of your eligibility to rebates on eyecare services and eyewear products.
