5. Customer must have a valid optical prescription. Customers may be required to undergo an eye test at their own cost prior to redemption of the offer if their prescription is not valid.
6. Package only available once per customer per year.
7. Selected frame options (e.g. $149) is based on RRP, without any discount or rebate and subject to availability and suitability.
8. Each customer may receive up to two new Stellest Myopia control lenses per calendar year from the date of first purchase (a “Refresh”), fitted to the original frames purchased. Lenses will only be Refreshed if the treating Optometrist is of the professional opinion and view (in their sole discretion) that a customer’s prescription has changed (from your last fitting with us) to the extent that new lenses are required (it must be a 0.50 diopter change or more to be eligible). This service is available to all customers of the Stellest Myopia control lenses, whether the Stellest lenses are purchased in a Myopia Management Package or otherwise. Lenses must be replaced into original frame. If an additional or new frame is required, additional costs may apply.
9. All purchasers of the Stellest Myopia control lenses will be offered two annual check ins, including customers who participate in the Myopia Management Package. The review consultations are designed to maximise the efficacy of the Stellest lenses and may include, subject to the professional opinion and view (in their sole discretion) of the Optometrists, assessing and updating your prescription, frame adjustments and general assessments.
10. Ultra-Wide Digital Retinal Scan and Axial Length Biometry are subject to availability at participating stores only. Our optometrists will provide advice in their reasonable discretion and professional opinion about each customer’s suitability for these scans.
11. 12 months free protection of your glasses is included in the Myopia Management Packages, provided under our Eyewear Protection Plan (provided by Virginia Surety Company, Inc.
